We are happy you are thinking of joining us. Registering is a simple process you can complete from home.
Whether you are registering yourself or your children, please follow the steps below to make sure your registration is processed without any delays.
Step 1: Adult Registration (Your Digital Handshake)
Think of our online form as your digital handshake with the surgery.
How to Register:

Check Your Postcode: Use our boundary checker (see below) to make sure you live within our catchment area.

Complete All Fields: It is essential that you fill in every single field. Forms cannot be processed if information is missing.

Have Your NHS Number: While you do not need ID to register, including your NHS number on the form makes the process much faster and more accurate for us.
Once you submit the form, it is sent securely to be processed. You will then receive a text message from us confirming that you are fully registered.
(Note: If you do not have access to the internet, please attend one of our surgeries and fill in a basic paper registration form).
Step 2: Registering a Child (Adding a Plus-One)
If you have a new baby or a child under the age of 16 to register, the process is very simple, but there is one vital rule to make sure it goes smoothly.
The Golden Rule: Register the Parent First
Before you can register a child, at least one parent or guardian must already be fully registered as a patient at Whaddon Healthcare. This is a crucial step to link your child's medical records correctly and safely.
How to Register Your Child:

Wait for Confirmation: If you have just registered yourself, please wait until you have received your confirmation text message.

Complete the Form: Once you are confirmed as a patient, go to the NHS portal and fill out a separate registration form for your child, in exactly the same way you did for yourself.

Confirmation: Once processed, you will receive another text message confirming that your child is now registered.

Accessing Someone Else’s Information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To request proxy access, please collect a proxy access form from Reception.
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
Accountable GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
