Risk Stratification Update
Update on Risk Stratification for Integrated Care Boards (ICBs)
In July 2022, when clinical commissioning groups (CCGs) were replaced with integrated care boards (ICBs) under the Health and Care Act 2022, Confidentiality Advisory Group (CAG) made an amendment to allow the continued processing of patient confidential data in line with the new Act by the ICB and data processors acting on behalf of GPs.
NHS England encourages ICBs and GPs to use risk stratification tools as part of their local strategies for supporting patients with long-term conditions and to help and prevent avoidable admissions. As responsible data controllers, Integrated Care Boards are required to undertake a review of their processing activities, update their own privacy notices in order to ensure transparency and transfer of data controller responsibilities.
In September 2023, CAG approved the BLMK ICB application, to continue risk stratification. As part of our application we are required to communicate to patients to advise them how their data is used and provide a way for patients to opt out.
Practices should continue working in the same way that they have always done for risk stratification and applying the code to patient records if they wish to opt out.
We have created a pack of information for patients, see attachments, we would appreciate your support with making the information available to patients. The information pack includes; a digital poster, an information leaflet to be shared online and website copy.

From 04/03/2024 our surgery will be implementing Accurx for all admin and prescription queries.
Accurx is a communication tool that is designed to link your contact with the surgery, directly to your records. It is an online service that allows you to communicate with the surgery easily, instead of waiting in a call queue.
Once the system is fully intergrated, patients will be able to request appointments, request medication, make a general enquiry, reply to messages and complete medical questionnaires.
From 04/03/2024 Whaddon Healthcare will be launching the admin function, for a quick demonstration please follow app.arcade.software/share/p630RPsnZVkSeUtWG7zc
