About Us
Our Service
We provide a specialist service to adult patients to support self management of their heart failure through education. We care for patients with Heart Failure with reduced and mildly reduced ejection fraction across the community of Milton Keynes and we conduct assessments in clinic settings in GP surgeries or in the patient’s own home if they are housebound.
We can support patients in the uptitration of their heart failure medications, with symptom management, and help to prevent hospital admission. We also support patients requiring heart failure palliative care in conjunction with other services including Willen at Home.
We are a non emergency service and operate Monday to Friday from 9am to 4pm.
Who We Support and How to Access Our Service
Our heart failure service is here to support patients who have been referred by a healthcare professional. Referrals come from:
- The Heart Failure Team at Milton Keynes University Hospital (MKUH)
- Your GP
- Cardiac Rehabilitation teams
- Oxford Heart Centre
Please note: we are unable to accept self-referrals. To be eligible, patients must have a confirmed diagnosis of heart failure, verified by an echocardiogram (heart scan) within the last five years.
Why You Might Be Referred
Patients are referred to us for help with:
- Understanding their condition and treatment options
- Adjusting medications to the most effective dose
- Managing fluid retention
- Receiving palliative care and support
What do we see in our clinics?
- We detect and treat early signs of clinical deterioration and reduce the need for unnecessary hospital admissions and re-admissions.
- We work alongside the heart failure team of Milton Keynes Hospital and the Community Cardiology service at Whaddon Healthcare.
- We optimise the management of people with chronic heart failure and aim to provide seamless care between hospital and general practice.
- Our approach is to actively encourage and support self-management. We identify and develop support mechanisms for both patients and their carers, assisting in improving their quality of life. We also aim to improve the end of life experience for patients and their families.
- We act as a resource for other healthcare professionals.
About Heart Failure
Heart failure means that the heart is unable to pump blood around the body properly. It usually happens because the heart has become too weak or stiff.
It's sometimes called congestive heart failure, although this name is not widely used now.
Heart failure does not mean your heart has stopped working. It means it needs some support to help it work better.
It can occur at any age, but is most common in older people.
Heart failure is a long-term condition that tends to get gradually worse over time.
It cannot usually be cured, but the symptoms can often be controlled for many years.
